InXite SmartCare™ is our cloud-based, patient-centered platform that enables our clients to successfully transition to value-based care. InXite SmartCare places the patient at the center of the system by providing a common platform for everyone in the patient's care team to view and contribute to one unified plan. Our technology works best with a dedicated patient care coordinator who interacts with patients monthly to ensure they are adhering to the care plan. The care coordinator also serves as a liaison between the patient and the physicians to keep everyone in constant communication.

Enhancement Modules

Users can enhance InXite SmartCare by adding additional technology modules, including quality and population management reporting, educational videos for patients, advanced messaging and alerts, telehealth, and virtual navigation.

The Blue Button is a nationwide program built by Centers for Medicare and Medicaid Services and the Department of Veterans Affairs that allows patients to view and download their personal health record in a PDF or plaintext file. Since its inception in 2010, more than 1 million people have used the Blue Button to access their information. 

MyHealthEData is a government-wide initiative that works to put patients at the center of the U.S. healthcare system by breaking down barriers that keep patients from accessing their complete healthcare record on the device or application of their choice. The initiative aims to empower patients by giving them more control over their healthcare data and allowing them decide how they want to their data to be used.

The Ohio Patient-Centered Primary Care Collaborative (OPCPCC) is a coalition of primary care providers, insurers, employers, consumer advocates, government officials and public health professionals. They are joining together to create a more effective and efficient model of health care delivery in Ohio. That model of care is the Patient-Centered Medical Home (PCMH).

Founded in 2006, the Patient-Centered Primary Care Collaborative (PCPCC) is a not-for-profit membership organization dedicated to advancing an effective and efficient health system built on a strong foundation of primary care and the patient-centered medical home. The PCPCC achieves its mission through the work of its five Stakeholder Centers, led by experts and thought leaders dedicated to transforming the U.S. healthcare system through delivery reform, payment reform, patient engagement, and benefit redesign.

The award-winning DEC began as a partnership between the City of Dublin and TechColumbus to provide services and promote an environment to increase deal flow for potential entrepreneurial and technology development resulting in more new businesses and job creation in Dublin and the surrounding area. The DEC combines an entrepreneurial center and data center. DEC tenants have access to the robust DubLink broadband network and, through the adjacent Metro Data Center, data storage in a highly secure environment.
